To make an appointment with our Student Clinic for a massage, please fill out and submit the Appointment for a Massage and Payment Form as well as the Health Information Intake Form and Pain Assessment Form. The Health Information Intake Form and Pain Assessment Form should be uploaded (inserted) toward the bottom of the Appointment for a Massage and Payment Form.
